-We are pleased to accept new patients of all ages.  Prior to your first visit, it would be helpful if you read all of the new patient information and completed the pre-visit forms found at the link below.  As we respect your time, this will allow us to expedite your initial visit with us and maximize the time during your appointment.

-We request that you let us know at least 24 hours in advance to not be charged for a scheduled appointment.
